REMINDER:  Reopening Requests for Modifiers KX, GA, GZ and GY

Please be reminded that beginning December 1, 2009, reopening requests to add, change, or remove modifiers KX, GA, GZ, and GY are no longer accepted.  Any request that you have to add, change, or remove these specific modifiers must be appealed through a redetermination request with proper documentation.

Please note that this change is effective based on the date of receipt of the request, not the date of service or process date of the original claim.  For example, if a claim for date of service 10/14/09 was originally processed on 11/12/09, and you submit a reopening request on 12/1/09, the request will not be accepted because the request was received by the Reopenings department on or after 12/1/09.

If a claim has been returned as unprocessable due to a missing KX, GA, GZ, or GY modifier (ANSI code CO-16, remark code M78), it must be resubmitted, not appealed.  For additional information, please see the article entitled IMPORTANT CHANGE – KX, GA, GZ and GY Modifiers – Claim Rejection.
